This is a Roman Catholic prayer to Jesus, developed in the medieval periods. It's authorship is unknown at the moment. Its content has Catholic concepts of the Eucharist (Communion), baptism, and the Passion of Jesus.

It was written in Latin, so here's an English translation of it:

Soul of Christ, sanctify me
Body of Christ, save me
Blood of Christ, inebriate me
Water from the side of Christ, wash me
Passion of Christ, strengthen me
O good Jesus, hear me
Within Thy wounds hide me
Suffer me not to be separated from Thee
From the malignant enemy defend me
In the hour of my death call me
And bid me come unto Thee
That with Thy Saints I may praise Thee
Forever and ever.
Amen.

I personally like this prayer and I try to pray it every afternoon.
